The patient is a 49-year-old male who presented in ED with burns that occurred about one hour ago. I was called in to examine this patient. Patient was about to burn brush, but, unknown to him, the brush was doused with gasoline and exploded when he lit it. His shirt was on fire and he was able to pull it off shortly after it caught fire. He is having trouble moving in right arm. The right arm is blanched and appears leathery and pale and the arm, hand, and fingers are swelling and he has difficulty moving his arm. He has circumferential multiple sites 3rd degree burns to the total right arm, including wrist, hand, and fingers, 9%. He was able to get his shirt off. He has 1st degree burns on his left hand and fingers, including wrist, and chest, 7%. The decision is made to perform escharotomy immediately on the right arm and hand. The patient is taken to the OR. General anesthesia with endotracheal intubation is administered. He has been on IVs with antibiotics since admission. After the administration of general endotracheal anesthesia, the burns on the right uppper extremity are debrided and flushed with 1% povidone-iodine
